Why Do Calluses Form, and How Can You Prevent Them?

Calluses form as a protective response of the skin to friction or pressure. They develop when the skin thickens in response to repeated irritation, often seen on areas such as feet and hands.

According to the American Academy of Dermatology (AAD), calluses are areas of thickened skin that occur as a result of repeated friction, pressure, or irritation. These thickened layers create a barrier to protect the underlying skin from damage.

Calluses arise primarily from prolonged or repetitive mechanical stress. The skin detects this stress and responds by producing more skin cells. This process can be broken down into a few key parts:

  1. Friction and Pressure: Continuous rubbing against surfaces or tight footwear can create friction.
  2. Response Mechanism: The body’s natural response to protect itself results in the production of additional skin layers.
  3. Healing Process: Over time, the excessive cell production leads to a hardened or thickened patch of skin.

Technical terms involved include ‘hyperkeratosis,’ which is the scientific term for the thickening of skin. Hyperkeratosis occurs when the outer layer of skin (the epidermis) produces too much keratin, a protein that forms the structure of skin.

The formation of calluses also involves the following processes:

  1. Keratinocyte Activity: Skin cells called keratinocytes multiply in response to constant pressure, increasing skin thickness.
  2. Barrier Function: The thicker skin serves as a barrier against further injury.
  3. Reduced Sensitivity: While protective, this overproduction can reduce sensitivity in the callused area, as fewer nerve endings are exposed.

Several conditions or activities can contribute to callus formation:

  • Improper Footwear: Shoes that do not fit well can cause friction, leading to foot calluses.
  • Occupational Hazards: Jobs that require repetitive hand movements, like manual labor, can result in calluses on the hands.
  • Certain Foot Shapes: Flat feet or high arches can distribute pressure unevenly, leading to calluses.
  • Sports and Activities: Activities like running or playing musical instruments can lead to localized friction resulting in calluses on fingers or feet.
